While operating a vibrating compactor, a worker was killed when the compactor tipped over,
crushing him between the roll-over-protective (ROPS) canopy and the ground. When he backed
up the machine, the left rear wheel went up a short incline and the machine became unstable and
tipped over. The worker was not wearing a seat belt. He fell, or jumped off the operating seat to the
ground, where he was crushed by the ROPS canopy. A seat belt was available, but it was not used.
Safe work practices:
- Ensure that workers receive adequate direction and instruction in the safe performance
of their duties.
- Use seat belts while operating mobile equipment.
- Review the operator's manual with
new equipment operators before
they operate equipment.
