The owner/operator of a gravel loading operation was re-installing a transmission on a front-end loader. He was working alone, raising the heavy transmission with two come-a-long winches. He was working below the transmission when one of the lifting devices failed. The transmission fell on him, fatally injuring him.

Prevention alert:

• Do not work alone on a potentially dangerous job.
• Ensure all lifting devices are in safe working order.
• Do not enter an area made hazardous by elevated objects unless additional safeguards are in place.
