A sports diver was hired for a commercial dive to recover a hatch cover lying in 60 ft. (18 m) of lake water. The diver descended with a rope attached to a small, hand-held shackle. The rope went slack approximately 5 to 10 minutes into the dive. The person tending the rope pulled it up to find the shackle missing. He tied on another shackle and re-dropped the rope where bubbles were last observed. There was no response. The diver was later discovered on the lake bottom. He died from drowning.


SAFE WORK PRACTICE
EMPLOYERS:
All commercial diving operations must comply with Part 24 of the Occupational Health and Safety Regulation. A diving supervisor and a standby diver must be on site. A safe and effective lifeline or float must be used. The diver must have adequate communications with the surface.

WORKERS:
You must follow the provided safe work procedures at all times.
