The excavator operator picked up the vibrator compactor and placed it inside the bucket. The compactor was secured inside the bucket by a hydraulic thumb. He shut off the unit and walked out on one of the excavator tracks to talk to the construction supervisor. He left the bucket containing the compactor suspended about 3 ft. (1 m) above the ground. For reasons unknown, the supervisor walked under the bucket. As he was under the bucket the hydraulic thumb released and the compactor tumbled out of the bucket, falling on him. The supervisor was fatally injured.


SAFE WORK PRACTICE
EMPLOYERS:
You must provide up-to-date written safe work procedures to all workers. These work procedures must cover the safe operation of unattended equipment.

WORKERS:
You must not leave the controls of equipment, such as excavators, unattended unless the equipment has been secured against inadvertent movement by lowering the bucket to the ground.
